Survey

A survey is a method of collecting qualitative and quantitative data using a series of open-ended and close-ended questions. Surveys may be conducted by mail, over the internet, or on the phone. A survey is similar to a questionnaire, but also includes the process of collecting and analyzing the responses to the questions.
